Your First Visit

 

New Patient Information Forms are required to be filled out and handed in to our office when you arrive for your first visit. You may print these forms by clicking on the Patient Forms link.  These forms will open as an PDF file format.

The policies and coverage plans of insurance companies change frequently. Sharing in the Mission of Mercy, ELLE Center for Breast Health provides care to anyone in need.   But, it is your responsibility to find out from your individual insurance carrier what your financial responsibility will be. Even if we are not participating providers in your network, you are likely entitled to out of network benefits. Please call your insurance provider to determine this. 

Occasionally, we encounter the need to schedule emergency patients experiencing problems of an acute nature. We do try to stay on schedule; however, there are instances when even the best of planning cannot control your waiting room time.  In the case of a long wait or unanticipated emergency, we will be happy to reschedule another convenient time for you.
